Please bring a list of all your medications each time you see the doctor. The best time to ask for prescription refills is during your regular visit. When you miss a visit, you may be asked to schedule another before medication will be refilled. New prescriptions require an office visit and will not be ordered by phone. You may request a refill at the Bever Center by calling (513)892-1888 or from the Middletown Center by calling (513)425-8305 during regular hours. Please call 5-7 days before you run out of medication and allow at least 2 days before checking with your pharmacy.
When you call please give your name, phone number, doctor’s name, spell the name of your medication, the Rx number on the bottle, and the name and number of your pharmacy. If all the information is not provided we will be unable to refill your prescription.
