PHS opens the new Middletown center.

PHS enters into a partnership with Butler Behavioral Health Services and Solutions Community Counseling Services to bring integrated care to more area residents.

PHS opens a new office in Wilmington, Ohio (Clinton County) to service the patients of Solutions Community Counseling Services.


PHS receives National Committee for Quality Assurance (NCQA) accreditation, a Level #3 Patient Centered Medical Home. At that time, PHS was one of only three centers in Ohio to receive this accreditation.

PHS's 'Mission Dental' Mobile Dental Van was presented to the community. The van was purchased with funds from US Health and Human Services and the Hamilton Community Foundation. Those funds covered the cost to purchase and refurbish the vehicle.


PHS was awarded a dental residency project and program by Lutheran Medical.

PHS takes part in the Butler County emergency management plan.

PHS purchases the 210 S. Second Street building in Hamilton, securing our place in the community.


PHS opens Express Care at the Middletown Community Health Center.

PHS is awarded grants from Interact for Health to open school-based health centers for Fairfield and Hamilton City Schools.

PHS receives an access grant from the Ohio Association of Community Health Centers to open a center in the Oxford community.
