School-based Health is becoming a popular model of care but was almost unheard of in Butler County until Primary Health Solutions opened its first School-based Health Center in 2016.  Since then, Primary Health Solutions has opened 3 School-based Health Centers with a 4th scheduled to open this year, and another scheduled to open in 2021.  In addition to these centers, Primary Health Solutions has proposed a new model of School-based care.

In addition to working with local school districts to construct brand new health centers within their districts, Primary Health Solutions will begin working with schools to provide services to their students at already existing PHS Health Centers.  This means, schools in close proximity to existing PHS Health centers will be able to transport students to and from the closest center for appointments.  This model of care will be called “School-linked services.”

Medical Health services are based on unique client need. Medical services offered include immunizations, sports/work/annual physicals, asthma or other chronic disease management and acute care for sick or injured students.  Vision Services include comprehensive eye examinations and prescribing and dispensing corrective lenses if necessary. Dental services include preventative exams, cleanings, X-rays, and corrective treatment as needed.  Behavioral Health providers are also available for students.

Primary Health Solutions is working with three school districts this school year for this model. Talawanda Local Schools, Madison Local Schools, and Northridge Local Schools.  The relationship with Northridge Local Schools is the first for Primary Health Solutions, in the Dayton area.

Students are transported and receive care only with parental consent.  On the consent form, parents can determine what services their child can receive and if they are able to be transported by the school district to and from appointments.  Without this consent form on file, students will not be seen.
