Our goal is to provide the highest quality dental care available in the community. Our Dentists, Hygienists and support staff are here to help you achieve good oral health.

*After regular business hours, on-call Providers are available at 513-454-1111 for emergency clinical advice only.

Arrival

If you are a current patient of the PHS dental practice be sure to arrive 15 minutes before your appointment time to fill out any necessary paperwork.

If you are new the practice you must arrive 30 minutes before your appointment time to complete the necessary paperwork. 

New patients will be required to fill out a Registration and Medical/Dental History form when they arrive.  If applicable, you are welcome to print the forms prior to your appointment and bringing the completed forms with you at the time of the appointment.  See printable forms below.

Our goal is to have you in the dental chair at the time of your appointment.

If you arrive late or at the time of your appointment you may need to reschedule.  If you know that you are going to be late, please call the dental department in advance at 513-454-1111.

Canceling an Appointment

If you must cancel your appointment, please call at least 24 hours prior to your schedule visit. Failure to do so will be counted as a missed appointment.

If you have two (2) missed/no-show appointments, we may not schedule you for any future appointment. You may call the schedulers to see what options are available. 

Dental Appliances & Procedures

Dental Appliances & Procedures

In order to keep the costs of services as low as possible:

  • Patients who need dentures and other custom made dental appliances must pay 100% of the cost of the dental appliance prior to PHS ordering the appliance. Payments made for custom dental appliance are non-refundable once the appliance is ordered.
  • Patients who are having crown and bridge work must pay 50% of the cost of the visit to place the appliance prior to the visit and the remainder will be due the day of the visit.
